Medication Administration
At this program our nurse will come to your home on a daily basis to assist you or your loved ones with administering medications such as: Oral medication, Insulin, Inhalers, Eye drops, Injections (Vit. B12, Biweekly or Monthly Psychiatric Injections) Etc...
Monitoring
We monitor blood sugar, blood pressure, weight, and any vital signs requested by the physician, ensuring all medication administration times, readings, and health data are accurately recorded through Electronic MAR for each patient
Reporting
Our nurse maintains ongoing communication with the pharmacist and treating physician, providing detailed reports on a daily or weekly basis. This ensures timely medication adjustments and personalized care based on the patient’s health status and medical needs
